Her pain was revered by the families of other victims who had emotional testimony on the last day of the five -week ornamental investigation of fatal stabs on April 13 last year.
This question tried to understand how a 40 -year -old Queensland man, who has a long history of mental illness, enters the popular Sydney Shopping Center on a busy Saturday afternoon, kills six people and injures 10 others, including a 9 -month -old baby.
The court bids from dozens of evidence (doctors, survivors, victims’ families, police) to find out how Australia can find such a tragedy again.
Elizabeth told the New South Wales (NSW) inspection court: “It seems that my daughter and five others have died of cumulative failures in a series of error systems.
Shopping Center Staving Shock Country
It was a gentle and sparkling afternoon when Joel Cauchi entered a huge shopping center, just a few minutes away from the most famous beach in Australia.
Local time (GMT) 15:33 Cauchi took a 30cm knife from the backpack and stabbed the 25 -year -old Dawn Singleton, the first victim.

In 2015, he complained about drug side effects, so Dr. Boros-Lavack began to reduce the dosage of clozapins used for therapeutic resistance schizophrenia after finding a second opinion from another psychiatrist.
She completely captured Clozapine in 2018, and Cauchi did not take medicine to treat obsessive compulsive disorders, she said.
For the first time in 2019, Cauchi no longer took anti -psychosis drugs. She admitted that Dr. Boros-Lavack had no second opinion on stopping the drug completely, and she was in question.
In most cases, the survey was heard from medical experts who said that patients out of anti -psychotic drugs turned to other treatments rather than stopping treatment.
In a few months, the mother of Cau Chi found a note that he believed that he was “under the control of Satan,” and contacted a psychiatrist for concerns about his son’s mental state. At the same time, Cauchi developed what Dr. Boros-Lavack said that it was “compulsive interest in pornography.” She wrote a prescription, but the survey said that it depends on Cauchi to decide whether he will take the medicine again.
In 2020, Cauchi left his family and moved to Brisbane and did not see Dr. Boros-Lavack.
After almost 20 years of treatment, Couchi had no regular psychiatrists, did not take drugs to treat schizophrenia, and had no family nearby.
The survey heard that he began to find the total license and contacted three Brisbane doctors to support the application for the medical certificate. They did not request access to medical files or Dr. Boros-Lavack did not receive their entire history. The third doctor gave Cauchi a later permission, but he never applied for a gun, the court said.

In 2022, Cauchi called the girl’s school and reported to the police and asked if students could swim and sports. The police officer tried to call Cauchi but could not contact him.
In January 2023, Cauchi moved with his parents at TooWOOMBA and complained that his father was stolen “Pygnife.” Currently, his mother said he should return the medicine with concerns with police officers.
The officials heard that the authorities could not detain people for mental health reasons if they were not in danger, and that police officers did not meet the explanation.
After the call out, one of the police officers who attended sent an email to the internal police mental health coordinator and asked Cauchi for follow -up. But I heard that this email was overlooked due to lack of manpower.
A few months later, Sydney’s police found that Kauchi received a call from the relevant passenger and then became rough near the road.
By 2024, Cauchi’s mental health was deteriorated, he was homeless and isolated with his family.
3 minutes changed everything

They found that Dr. Boros-Lavack missed the opportunity to return to anti-psychotic drugs, saying that he had not taken Cauchi’s mother’s mother’s concern at the end of 2019.
The panel also provided evidence in the survey of “floridly psychotic” in the active part of the psychosis episode when Cauchi entered the shopping center.
Dr. Boros-Lavack emphasized: “I did not fail to take care of Joel.”
She said before that Cauchi was not a psychotic during the attack and that medication did not block the tragedy.
Dr. Boros-Lavack said that this attack is “because of his sexual frustration, hatred of obscene and women.”
But the next day, she withdrew the evidence and simply said it was “guessing” and was not a position to evaluate the mental state of Cauchi who had not treated him since 2019.
However, the survey is investigating whether Cachi has targeted a specific individual or group.
The answer was clear for Jade’s brother Peter Young. “His Rad Hunt found 16 victims, saying that he was frustrated without finding a good girl to marry, and 14 of them were women.”
